The appropriate adaptations to assessments can be to have the students work in groups or help set-up the computers for the appropriate software that will help the student to reach 
their assessment goals appropriately.

The directions and procedures are explained throughout the lesson. Scoring can be explained in a rubric so the students will understand what is expected from the individual or group work that is to be turned in at the end of the lesson.
The assessment plan can include multiple assessment modes depending on the lesson that is being presented. If the lesson is incorporating many uses of the technology then the students can be assessed throughout the instructional on how will they are comprehending the software, if they would be able to find the software again if asked to perform the same task, and what uses the software being presented will be used good for in home or for future assignments.
Assessment and goals are linked in multiple ways, they both help the teacher to grasp a better understanding of where the students are within the lesson. The goals are set at the beginning of the lesson to help the students understand what is to be taught and comprehended. The assessment is throughout the lesson or at the end to help the students see what they are learning or have learned throughout the lesson.
Learning goals are assessed at the beginning of the lesson, during the lesson, and at the end of the lesson. They help to provide an understanding of the lesson that is being taught to the students so they (students) will be able to comprehend what the lesson is teaching them.
